Exercise Physiology for Aged Care Clients

Exercare provides accredited exercise physiology to older Australians living at home or in residential aged care across south-east Melbourne. We deliver mobile visits to your home, community venue, or facility — eliminating the need to travel to a clinic, which for many older clients makes all the difference to consistent participation.

Our exercise physiologists design evidence-based programs that improve strength, balance, mobility and functional independence — helping older adults remain active, confident and living safely in their own home for longer.

Funding: Exercise physiology is an approved allied health service under Home Care Packages (HCP) Levels 1–4 and the Commonwealth Home Support Programme (CHSP). We liaise directly with your care manager and provide all required documentation.

Funding options

Home Care Package & CHSP Funding

Exercise physiology is an approved allied health service under all levels of Home Care Packages and the Commonwealth Home Support Programme. If exercise physiology is included in your care plan, Exercare invoices your provider directly — you do not pay out of pocket.

Home care packages (HCP)

Available at all levels (1–4). We work with your care manager to include EP in your care plan and provide all required progress notes and reports.

CHSP

The Commonwealth Home Support Programme covers allied health including exercise physiology for eligible older Australians. Contact My Aged Care on 1800 200 422 to check eligibility.

Aged Care Exercise Physiology FAQs

Can I use my home care package for exercise physiology?

Yes. Exercise physiology is an approved allied health service under HCP Levels 1–4 and CHSP. Exercare invoices your care provider directly and provides all required documentation.

Do I need a GP referral for aged care exercise physiology?

No referral is required for HCP or CHSP-funded clients. A GP referral is only required if accessing sessions through Medicare's Chronic Disease Management plan.

Will you visit my loved one in a residential facility?

Yes. Exercare visits residential aged care facilities across south-east Melbourne. Please contact us to discuss your facility's requirements and visit arrangements.

How does falls prevention exercise physiology work?

Your exercise physiologist conducts a falls risk assessment, then designs a personalised program including strength training, balance exercises, gait training and functional movement. Programs are progressive and adapted as your strength and confidence improves.
